Position within the company: Receives hierarchical direction from the Director of Care. Provides functional instructions to fellow careers and nurses regarding wound care. The wound nurse works organization-wide.
Tasks
Purpose and core of the function
Optimizing the quality of wound care for clients of Geraszorg in their own living environment, so that the quality of life of the clients improves as much as possible and self-reliance is promoted. Guiding the client and his relative(s) for optimal wound care and maintaining contact with the practitioner and/or other disciplines. The wound nurse advises and supports the employees with (complex) wound care. The wound nurse is expected to contribute to policy development of wound care.

- Optimizing the quality of wound care for clients of Geraszorg in their own living environment, so that the quality of life of the clients improves as much as possible and self-reliance is promoted. Guiding the client and his relative(s) for optimal wound care and maintaining contact with the practitioner and/or other disciplines. The wound nurse advises and supports the employees with (complex) wound care. The wound nurse is expected to contribute to the policy development of wound care.
- Drawing up a treatment plan based on the diagnosis in consultation with the client and the attending physician.
- Supervising the adequate implementation of the treatment plan.
- Advising and providing information and instruction to the client and his relative(s) with respect to wound treatment and the care process.
- Guiding and encouraging clients to become more self-reliant with regard to wound care.
- Adjusting the treatment plan on the instructions of and signaling by team members.
- Identifying and registering changes in the health and/or living environment of the client, adjusting the treatment plan accordingly if necessary.
- Directing (coordination, organization and evaluation) in the complex wound care around the client.
- Performing technical nursing and reserved actions in (highly) complex situations in accordance with the protocols (recognized) by Geras Caregivers for which authorization and competence are required.
2. Advice, education and training for employees
- Answering questions, advising and supporting employees in (complex) wound care.
- Giving instructions to employees with regard to nursing procedures with regard to wound care.
- Supporting the use of guidelines, protocols and tools for the necessary wound care.
- Providing information/education and transfer of knowledge in the field of wound care to employees.
- Developing/adjusting a training program on wound care.
- Informing employees about new developments with regard to wound care.
3. Monitoring and promoting the quality of wound care
- Monitoring and signaling external developments in the field of wound care.
- Contributing to the policy development of wound care.
- Identifying bottlenecks in the implementation of wound care and solving them.
- Participating in projects related to the development of wound management.
- (co-)monitoring and promoting the quality of wound care, cooperation with other disciplines, informal care and the client with regard to the organization of wound care.
4. External environment
- Maintaining contacts and coordination with the attending physician or specialist about the progress and course of the wound treatment.
- Participating in networks with colleagues and maintaining contacts within those networks.
- Looking for opportunities to collaborate with other colleagues and/or disciplines within the regions.
- Profiling Geras Caregivers in the delivery chain.
- Other activities
- The (incidental) performance of district nursing activities such as wound policy and/or drawing up and evaluating a care plan.
- If necessary, (support with) executing a transfer to another institution.
- Reporting to other care providers involved.
Requirements
JOB REQUIREMENTS
Knowledge
The position requires completed training as a nurse at HBO level (Bachelor Degree) and a valid registration in the Dutch BIG register and extensive work experience in wound care. In addition, a post-Bachelor Degree training for wound nurse has been followed. The position also requires knowledge of the organisation, policies and procedures, of the healthcare product, the regional social map and relevant laws and regulations. Is aware of developments in the field, keeps track of them and assesses their applicability within Geras Caregivers. Knowledge of computer skills is necessary. Has analytical skills to see connections and make the correct diagnosis..
Independence
The position requires that content is given to the position independently, activities are planned and initiatives are taken. The activities are carried out within the annual plan, within the budget and on the basis of the frameworks and protocols. Is able to respond to changes in the client’s health situation and living environment and adjusts the wound plan. Non-ordinary and complex problems in the field of wound care are solved independently. It is possible to fall back on the Care Director.
Social skills
In direct care, there is communication with clients and/or family and relatives. This requires tact, empathy, helpfulness, flexibility and a respectful attitude. Social skills are required when advising and training healthcare workers. Is able to establish a functional relationship with the client, informal caregivers and other care providers. Can collaborate with other disciplines. Persuasiveness is important in stimulating and motivating clients in their self-reliance and in giving advice.
Risks, responsibilities and influence / Span of control
There is a risk of causing bodily harm to clients when performing nursing and reserved and risky actions. Providing remote advice to employees increases the risk of making an incorrect assessment of the client’s situation, which can have serious consequences for the client. The other activities involve a risk of material and financial damage. There is a risk of causing immaterial damage in contact with clients, informal caregivers and other care providers. Geras Caregivers may be indirectly harmed as a result. The officer ensures a positive image of Geras Caregivers when profiling in the chain. They influence the quality of care and are responsible for making a correct diagnosis and a treatment plan which can have serious consequences for the client. The other activities involve a risk of material and financial damage. There is a risk of causing immaterial damage in contact with clients, informal caregivers and other care providers. Geras Caregivers may be indirectly harmed as a result. The officer ensures a positive image of Geras Caregivers when profiling in the chain. They influence the quality of care and are responsible for making a correct diagnosis and a treatment plan.

Good oral expression skills are required in connection with communicating with clients, informal caregivers, colleagues and other care providers. Written expression skills are required for writing wound policies, treatment plans and reports in both internal and external contacts. In addition, written expression skills are required for writing improvement proposals with regard to the wound care policy

Attentiveness is necessary during the performance of nursing activities and reserved and risky actions in rapidly changing circumstances, when stimulating and motivating clients and when monitoring the quality of care. Identifying and taking action is required in the event of changes in the client’s health and living environment. Attention is important when signaling developments in the field. Inattention has consequences for the progress and quality of one’s own work and for the client, colleagues and others in the client’s living environment.
Other job requirements
Perseverance and perseverance are important when supporting the client and their loved ones and when advising colleagues. Methodical and orderly work is important in arriving at a correct diagnosis and treatment plan. Hygiene is important when caring for clients. Aggravating circumstances
Physical strain can occur when nursing clients in unequipped and changing working conditions. There is psychological stress due to the confrontation with serious, sometimes prolonged and hopeless suffering and dealing with resistance and opposing interests. There are aggravating working conditions and/or risk of personal injury due to working with hypodermic needles, excretion products and cytostatics. Carrying out caring and nursing work and contacts with aggressive/confused and infectious clients can pose a risk of physical injury.
